Information Regarding the Prevention of Swine Flu
H1N1 Clinic Schedule for Saturday, January 23, 2010 at the Samsel Upper Elementary School
9:00 -10:00 a.m. – Dose #1 for Arleth, Eisenhower, Truman, and Wilson students who registered for the single injectable dose of H1N1 with trace elements of Thimerasol.
10:00-11:00 a.m. – Dose #1 for SUES, Middle School and High School students who registered for the single injectable dose of H1N1 with trace elements of Thimerasol.
11:00 a.m.-12:00 p.m. – Dose #2 for Selover and SUES students under 10 years old who received Dose #1 at one of our previous H1N1 clinics. Dose #2 will be the same injectable or nasal mist as Dose #1.
12:00-1:00 p.m. – Dose # 2 for Arleth and Eisenhower students who received Dose #1 at one of our previous H1N1 clinics. Dose #2 will be the same injectable or nasal mist as Dose #1.
1:00-2:00 p.m. – Dose #2 for Truman and Wilson students who received Dose #1 at one of our previous H1N1 clinics. Dose #2 will be the same injectable or nasal mist as Dose #1.
2:00-3:00 p.m. – Dose #1 for any student who registered but missed their original scheduled date for Dose #1. Dose #1 for any Sayreville Public School employee who registered, regardless of qualifying medical condition. Vaccine will be administered as long as the supply lasts.
REQUIREMENTS FOR ALL STUDENTS ABOVE: 1) Parent who signed the registration form MUST accompany the student and have a picture ID. 2) Students MUST wear a sleeveless shirt under a button front shirt/sweater.

SATURDAY, JANUARY 23, 2010.
Single injectable doses of H1N1 vaccine with trace elements of Thimersol are now available for students whose parents wanted that form only. See the Saturday, January 23rd H1N1 Clinic Schedule above on this website.
If you will not be bringing your child, call your school nurse, unless you have previously notified the school that you no longer want the vaccine.

H1N1 CONSENT FORM TO VACCINATE STUDENT AND H1N1 INFORMATION SHEET - DUE TUESDAY, NOVEMBER 10, 2009.
If you do not submit a consent form by Tuesday November 10, 2009, your child will not be eligible for the H1N1 vaccine. Print the Consent Form as a 2-sided document , complete the information and return to the Homeroom Teacher.
